A request for a hearing shall be sent to the official representative named under §10.818(f) and shall contain: (a) A concise notice of the issues on which the provider desires to give evidence at the hearing; (b) Any request for a more definite statement by OWCP; (c) Any request for the presentation of oral argument or evidence; and (d) Any request for a certification of questions concerning professional medical standards, medical ethics or medical regulation for an advisory opinion from a competent recognized professional organization or Federal, State or local regulatory body.
