29 C.F.R. § 801.52 Contents of notice.
Title 29 - Labor
The notice required by §801.51 of this part shall: (a) Set forth the determination of the Administrator and the reason or reasons therefor; (b) Set forth a description of each violation and the amount assessed for each violation; (c) Set forth the right to request a hearing on such determination; (d) Inform any affected person or persons that in the absence of a timely request for a hearing, the determination of the Administrator shall become final and unappealable; and (e) Set forth the time and method for requesting a hearing, and the procedures relating thereto, as set forth in §801.53 of this part.
