Title 41 - Public Contracts and Property Management
You must include: (a) The name of the employee(s); (b) The signature of the proper authorizing official; (c) Purpose of travel; (d) Any conditions of or limitations on that authorization; (e) An estimate of the travel costs (for open authorizations it should include an estimate of the travel costs over the period covered); and (f) A statement that the employee(s) is (are) authorized to travel.
